Can a Nurse Practitioner be a Medical Director in Manitoba?

Yes, and in Manitoba the Nurse Practitioner is the key to it. An NP can be the authorizing prescriber for a med spa or a nurse-led injectables practice without being on the premises. A physician can only direct a clinic they actually practise in. So the practices asking us for help across Manitoba need NPs, and we do not have enough of them.

This is general information, not regulatory advice. Physicians in Manitoba are regulated by the College of Physicians and Surgeons of Manitoba (CPSM) and nurses, including Nurse Practitioners, by the College of Registered Nurses of Manitoba (CRNM). We checked both colleges' standards in August 2026. Confirm what applies to you with your own college before you take on a practice.

Why Manitoba is an NP province

Under CPSM rules a physician cannot be the Medical Director of a clinic they do not practise in. That rules out the arrangement most other provinces rely on, where a physician oversees a nurse-led practice from elsewhere. A Nurse Practitioner is not caught by that rule. An NP registered with the CRNM can assess clients, prescribe and authorize treatment within their scope and competence.

The practical result: for a solo nurse injector in Winnipeg, Brandon or Steinbach who does not have a physician working in their clinic, an NP is the route to practising properly.

What the work is

  • Assess each client and issue a client-specific order. A telehealth assessment is acceptable. A standing order with nobody assessing the individual is not.
  • Keep the practice's orders and protocols current for the treatments it actually offers.
  • Review charts and be satisfied with consent and documentation.
  • Be reachable for complications, including a suspected vascular occlusion.

This is genuine clinical oversight with your name and registration behind it. If an arrangement only wants a signature, it is the wrong arrangement.

If you are a Manitoba physician

You can still be the Medical Director of a clinic where you practise. If you work in, or are willing to work from, an aesthetic clinic, tell us: some practices want exactly that. What you cannot do under CPSM rules is direct a clinic you never practise in.
